In compliance with Texas House Bill 4224 (89th Regular Session) and Section 181.105 of the Texas Health & Safety Code, Bloom for Moms provides the following information to help consumers access their health care records, contact the licensing authority, and file a consumer complaint.

 

 

1. How to Request Your Health Care Records

 

You have the right to request access to your health care records maintained by this practice.

 

To request your records:

 

• Submit a written request that includes:

   

  • Your full name

  • Date of birth

  • Preferred contact information

  • Dates of service

  • A description of the records you are requesting

As a telehealth-only practice, all records requests are handled electronically. Requests will be processed in accordance with Texas law and federal privacy regulations (HIPAA) within the timeframes allowed by law.

3. File a Consumer Complaint

 

If you wish to file a consumer complaint, you may do so with the Office of the Attorney General of Texas – Consumer Protection Division.
